We offer PT packages at reduced rate of $525 for a 4-pack of self pay PT visits (equal to $131.25/visit)

 INSURANCE

We are only in network with specific Anthem BCBS PPO plans. A co-pay will be expected before services are rendered. Please contact us to verify if we are in network with your plan.

Patients with Tricare benefits: we accept Tricare but are out of network. You may have an out-of-network deductible that you will have to reach. At that point, you will pay 20-25% coinsurance based on your plan, and we will file all claims for Tricare based on our agreement with them. Please call with questions and check your benefits with Tricare.

To allow us to continue to offer high level, one-on-one individualized care, we are not in network with all insurance companies.

We do accept Care Credit and FSA/HSA payments.

While we may not deal directly with your insurance, we work with many insurance networks as an out of network provider. For your convenience, we will provide you with a superbill for you to submit to your insurance if you are out of network. *We collect a quick pay (cash) rate up front and the reimbursement from the insurance company would go directly to you, the patient. If you are out of network and plan to seek reimbursement, we recommend calling your insurance company and asking for information on out of network benefits.  You will want to know:

1. If your out of network deductible has been met for the year

2. Your co-insurance after the deductible has been met

3. Annual visit limit

4. Any authorization that may be required

We offer the competitive quick pay cash rates above as an option for those coming to us via direct access (not with a physician’s referral) or who choose not to submit to insurance.

We are non-participating providers with Medicare, however; we will file claims directly on your behalf.  This means you will be responsible for payment up front, and Medicare will send reimbursement directly to you once the claim is processed.

DIRECT ACCESS: SEE US WITHOUT A PHYSICIAN REFERRAL

For most insurance reimbursement, a physician’s referral is required, however; Virginia is a direct access state.  That means you are able to come to physical therapy with no delay in care, without obtaining a referral from your provider.  We are also now able to perform dry needling services without a referral. We prefer for you to have a referral if you are pregnant.

FAQ’S 

  • Yes. We will email you paperwork 1 week prior to your initial evaluation. Please email that back ahead of time or arrive 15 minutes early to fill it out before your initial evaluation.

  • Yes! We are child friendly. We will be happy to accommodate your infant/child with you at your visit with the understanding that this is your time to heal. If we find that over multiple visits we are not able to progress forward appropriately because your child is present, we may ask you to consider childcare options during your visit. If your child is sick, please keep them home.

  • We ask for a 24 hours notice for all cancellations including initial evaluations. We do this because we allow for 1 hour of individualized care for you. You must cancel either by leaving a voicemail, talking to a staff member, or emailing us. If you fail to provide a 24 hours notice, you will be billed a $60 cancellation fee. No shows will be billed the entire visit charge. If you no show 2 times, your remaining visits will be taken off the schedule.
